Doctor’s note

Hi everyone,
I sent a link to every stores email with a document to use if a patient needs a doctors. Any store that has a doctor that is not apart of Apex has their own document with the H&L logo. If there is already something being used for a particular doctor/office and it has been working well feel free to continue using what works. This was just made to help anyone stores if there was ever a need for this document.
