MEDICAL INSURANCE RENEWAL

Good morning,
I just faxed over the renewal forms for the medical insurance to all the managers. If your store did not receive the fax, please let me know.
I only need the copy of the application or the waiver. Feel free to fax it to
me @ 860-828-7406. Just a reminder, for those employees signing up for the insurance – you DO NOT need to complete the medical history – page 3.
